Our office is dedicated to the practice and teaching of preventive dentistry. Please be assured that you will receive the most thorough and conscientious dental service we can provide.

About Dr. Maxwell

Dr. Maxwell has been in the private practice of General Dentistry since 1970. He is a graduate with distinction from the University of Nebraska College of Dentistry. His professional memberships include the American Dental Association, the Texas Dental Association, the Greater Houston Dental Society,the Academy of General Dentistry and Omicron Kappa Upsilon, national dental scholastic honorary.


Your First Visit

Upon arrival you will be asked to complete a patient information form which will include pertinent information concerning your health.

Diagnosis is the key to preventive dentistry. So, our clinical procedures will include examination of your head and neck, facial and oral soft tissue, oral cancer screening, dental examination and periodontal evaluation. We also require a panoramic radiograph of all your teeth and jaws in addition to other diagnostic films.

Completion of these procedures will help us develop a treatment plan for you that you will understand, appreciate and can afford to complete.


Continuing Care

Regular preventive oral care is a wise investment.

We hope you will share our belief that early care is easier and less costly than treatment delayed. This is why we custom design and encourage regular preventive maintenance visits. These visits and their frequency will fit your particular dental health requirements.

Continuing care visits consist of oral cancer screenings, detection of decay and periodontal disease as well as preventive cleanings.



There is never a "right time for a dental emergency." We will make every effort to see you ASAP.

Financial Considerations

Charges for services are due at the time dental treatment is provided.

We accept American Express, Discover, Master Card, Visa.

We also provide several methods of extended payment. You may inquire about these plans or pick up the information/application at our front desk.

You are responsible for your scheduled appointment and we require at least 24 hour notification from you when appointments must be changed. Failure to extend this courtesy to our office may result in a charge for the open appointment time.



Dental insurance is intended to cover a portion of your dental treatment charges. The amount of coverage depends on the individual contract your employer or union has with the insurance carrier. This coverage should be considered reimbursement for the fees incurred. This reimbursement is dependent on many factors including deductibles, maximum allowable benefits, substitution clauses and exclusion clauses. Our office will contact your carrier and get a breakdown of your dental benefits, but because the insurance carriers will not guarantee the coverage, we can only provide an estimate. Ultimately, the patient is responsible for all fees not covered by insurance.

We also accept any indemnity policies and PPO's who allow for out of network benefits. We do not participate in any managed care plans.

We want you to understand the type of insurance you have along with its benefits and limitations. If you are not sure what type of policy you have, please call the office. If we do not already know the answer to your questions, we will be happy to contact your carrier and discover that information for you.
